Outpatient Program

The Outpatient Pulmonary Rehabilitation Program assists patients in achieving their optimum quality of life by teaching the skills necessary for self management of their condition. The program's multidisciplinary team, including physicians, nurses and social workers, promotes behavioral skills and techniques tailored for each patient throughout their outpatient program.

Program steps:

  1. A patient is referred to the program by either their primary care doctor, their pulmonologist, their sub-specialist or self-referred.

  2. The patient completes required pre-program evaluations.

  3. The patient meets with respiratory therapists and physical therapists before starting the program.

  4. The patient completes a personalized goal statement with the multidisciplinary team.

  5. The patient receives frequent reassessments and re-examinations of goals.

  6. Providers complete discharge summary including a summary of patient's progress, team recommendations and exercise prescription for patient upon completion of program.

  7. The Pulmonary Rehabilitation Program will send notes and discharge summary to the patient's referring physician.

  8. The patient is referred to a continuity/maintenance program if desired and appropriate.
